In coordination with the Department of Defense, VA Liaisons ensure the seamless transition from active duty to VA health care for injured service members.  

At Military Treatment Facilities

VA Liaisons stationed at Military Treatment Facilities (MTFs) support the transfer of severely wounded service members. They coordinate care and provide consultation about VA resources and treatment options. Liaisons will contact the service member's local VA Medical Center and Transition Care Management (TCM) Team to ensure that appointments and care plans are in place before the service member leaves the MTF.

At Warrior Transition Units

For less severely injured service members, VA Liaisons are stationed with Army Warrior Transitions Units (WTUs) to coordinate ancillary care through their local VA Medical Center. WTUs provide personal support to wounded Soldiers who require at least six months of rehabilitative care and complex medical management.
