Every VA Medical Center has a Transition and Care Management Team ready to welcome Post 9/11 Combat and Non-Combat Veterans Home to help coordinate your health care. Case Managers, who are either nurses or social workers, coordinate all patient care activities, needs, and to help you navigate your way through the VA system.
A Transition Patient Advocate (TPA) acts as a personal advocate as you move throughout the VA healthcare system. To contact the Transition and Care Management Team, check your local VA medical center web site. You can find the website using the VA Transition and Care Management Team Locator.
